About the Tools

Guides reflect the phases of staff development and are paced to complement supervisor’s cultivation of staff and provider teams. These tools are not limited to the suggested phase of introduction as development is ongoing.

Checklists essential for the staff orientation to family teams and communicate foundation for supervisor & new staff:

  • Team Process Review Checklist
  • Supervisor Review of Care Coordinator

Checklists with greatest value for assessing and cultivating staff skills.  These can be completed by staff as self-assessment tools and reviewed with supervisor to compare observations.  Ideally suited for planning around staff strengths and needs:

  • Plan of Care Quality Review
  • Scoring Tool for Coaches
  • Team Process Reflection Tool
  • Care Coordinator and Supervisor Competencies*

Checklists that support the cultivation of provider teams.  These guides are developmentally informed to assist supervisor’s planning of staff capacity to safely serve complex families.

  • Developmental Stages of Facilitator
  • Developmental Stages of Supervisors
  • Risk/Staff Experience Grid

*WI SOC Care Coordinator and Supervisor Competencies developed by the Children Come First Advisor Committee and DHS to support best practice of wraparound in Wisconsin.  This tool can be used to communicate with local coordinating committees and human service boards the relationship between wraparound principles and the ongoing work of staff recruitment, onboarding and development.
